The Community Care Access Centre (CCAC) also provides information and assistance in arranging for placement in a long-term care centre. CCAC staff will help to facilitate the placement process and guide patients and their families through each and every step. Patients being discharged from hospital will receive priority status and are placed in the first available bed as per Ministry of Health and Long Term Care policy. CCAC staff can be reached in the hospital by calling ext. 2078, or can be contacted from outside of the Hospital at 1-888-470-2222. Once a patient has been designated as appropriate for an Alternative Level of Care (no longer requiring care in an acute care hospital), a co-payment fee will be charged while the patient is in hospital until the time of discharge to a long-term care centre.
