About the Service
The Psychogeriatric Assessment and Outreach Service is a comprehensive assessment and consultation service to physicians in York Region for those 65 years and older experiencing mental health difficulties.
This is an outpatient service within the Mental Health Program provided by professionals from psychiatry and nursing. The service is provided to seniors who are experiencing psychiatric symptoms and social adjustment problems. |
Outreach and Assessment
This comprehensive service consists of outreach to homes by a registered nurse followed by a consultation with a Psychiatrist in the Shaw Clinic.
The Outreach to homes is initiated by a phone contact with the clients/family to schedule an appointment. The home visit consists of a complete assessment including family history, medical history, mental health and social history. Any changes in behaviour, mood, general health that have occurred recently are recorded.
This is followed up by a consultation with a Psychiatrist in York Central Hospital’s Shaw Clinic located at the Jacob Keffer Site. The consultation report and recommendation are then forwarded to the referral source.
Short term follow-up services are offered as indicated.
A Collaborative Approach to Care
The Psychogeriatric Assessment and Outreach Service Team work in conjunction with services provided by family doctors, other health care providers and community supports.

The Psychogeriatric Assessment and Outreach Service accepts physician referrals only. Physicians may make a referral by calling 905-832-1406 ext. 2304, or faxing completed Referral forms to 905-832-1952.

Just minutes from Highways 400, 404 and 407, this location is at 37 Jacob Keffer Parkway in Vaughan. Close to Richmond Hill, Aurora, King City, Markham, Thornhill and Woodbridge, the Jacob Keffer Site is just off Rutherford Road, east of Keele Street.
