Geriatric Assessment Treatment Service (GATS)
This is an in-patient service for older person with complex medical conditions who require individualized assessment and rehabilitation. Patients receive assessment and treatment. Individual plans are developed for each patient based on personal goals and abilities. The average length of stay is 4 to 6 weeks. This Service is provided on level #1 and level #3 of the Centre for Continuing Care and Rehabilitation (CCCR) depending on individual care needs.
Philosophy of Care
Care has been developed in which a complete assessment of the physical, functional, psychological, social and spiritual status of each elderly individual is fundamental.
Assessment, prevention, planning and intervention, aims to optimize the independence of the elderly individual as they experience health problems.
A comprehensive assessment of the individual requires the coordinated skills of an interdisciplinary team.
The team, having assessed the individual, then ensures that the individual/family participates in a coordinated and comprehensive plan of care.

To provide a comprehensive geriatric assessment. |
To improve the individual's functioning, maximizing abilities.
To optimize the wellness of the elderly individuals and improve their quality of life.
To formulate an interdisciplinary patient centred treatment plan.
Who We Serve:
To be considered for admission, patients must have one or more of the following:
- Multiple, complex medical conditions;
- Worsening of medical problems;
- Need for further evaluation;
- Need for assessment and treatment related to change in functional status.
Our patients are:
- Generally 65 years of age or older;
- Are medically stable;
- Have defined rehabilitation goals;
- Have the desire and ability to actively participate in a rehabilitation program;
- Have the potential to return to some form of community living.
What to Bring
We suggest that you bring the following items to ensure your comfort:
- Personal care items such as soap, shampoo, lotion, toothbrush, tooth paste, kleenex,
electric razor. These are not supplied on the unit.
- Comfortable flat-soled shoes
- Easy dressing clothing such as track suits and sweat shirts
- Any assistive devices used prior to admission
- Personal items such as pictures and photographs
Discharge Guidelines
When you are ready for discharge, you are responsible for arranging discharge either home or another private arrangement. Please note: there are costs associated with private arrangements. The patient will not be able to stay in the GATS unit to wait for alternate living arrangements.
Patients awaiting discharge may be reviewed for participation in the Brookside Partnership Program. This service offers long term care levels of services at Brookside Retirement Residence for York Central Hospital patients awaiting discharge.
Accommodation
Both private (one bed per room) and ward (two beds per room) accommodation are available. Accommodations offered will be based on request and availability. If private accommodation has been requested, the patient will be charged the private
accommodation rate from the date of admission. Otherwise the patient will be moved to a two-bed room as soon as one becomes available. Information about cost of a private accommodation is available by calling the Client Liaison at (905) 883-1212 ext. 3809.
Please note: York Central Hospital is unable to obtain information about your insurance benefits. Please ensure that you know your insurance coverage. Rehabilitation and Complex Continuing Care coverage differs from regular hospital insurance coverage. Costs incurred beyond your health care coverage will be billed to you directly.
Your Care Team
An interdisciplinary team of healthcare professionals provides the Continuing Care Program services. Your care team includes you and your family/caregiver. For more information, please refer to the Continuing Care Program brochure or hospital
directory located in the patient room.
Meal Times
Meals are provided in the dining room as well as the patient room, depending on the patient's condition.
Television and Telephone
Telephones are located at the patient bedside.
For level # 1, television access and to enquire about costs, please call 905-821-9950. This service is available weekdays from 8:30 a.m. to 5:00 p.m.
On level # 3, patients must bring their own television sets and arrange for cable hook up from Rogers Cable. Information and telephone numbers are available at the Communication Centre.
Visiting Hours
Daytime visiting hours are flexible. Evening visiting hours end at 8:30 p.m.
