Patient and Family Information

York Central Hospital’s Rehabilitation unit and has 32 beds and the unit specializes in orthopaedic and general rehabilitation.

We are a short-term service for patients requiring 10-14 days of in-hospital rehabilitation. Our team will work closely with you and your family to establish goals and help you to return to the community.

Who We Serve
Admission to this area may be from within the hospital or from the community. If you are having a planned surgery at York Central Hospital, an application will be submitted from the Pre-Admission Clinic.

A physician will be assigned to follow you throughout your stay. We ask that you start now to think about goals you want to achieve before you leave the hospital, as well as, what assistance you may need when you return home.

About Your Care
It is important that you are involved in your plan of care to reach your goals. Some of your Rehabilitation Team will be coming to visit you at your bedside to discuss with you what you wish to achieve while you are here. Postings of days and time of bedside rounds are available on the unit.

The team will involve you and your family/caregiver in therapies, which promote independence, such as dressing, bathing, toileting, leisure activities and mobilization. Equipment needs will also be reviewed.

If your medical condition changes, you may need to be transferred to the acute care area of the hospital.

If you need help, a nurse is available 24 hours a day to:

  • coordinate your care;
  • give your medications;
  • assist you as needed with our day-to-day care; and answer your questions or forward them to the right person for you

Please let the nurse know of any medicines you are taking. This includes Rolaids, cough drops, or any herbal medicines, vitamins. Please note that there may be additional costs that are not covered by OHIP i.e. splints, support stockings and/or crutches.

Patients wanting to leave the unit (ie: to visit the outdoors) are asked to sign the clipboard at the nursing station and let the nurses know prior to leaving.

Therapy
The rehabilitation gym is located on Level 1 (ground floor) of the CCCR’s East Wing. Therapy will be provided 5 to 6 days a week. Therapy on the weekend will depend on your individual needs as assessed by the rehabilitation team.

Mealtimes
All meals are served in the dining room. Traveling to and from the dining room will become part of your therapy. Most patients find the dining room experience to be an enjoyable opportunity to socialize.

Meals are served at:

Breakfast 7:45 a.m. to 9:00 a.m.
Lunch 11:45 a.m. to 1:00 p.m.
Supper 4:45 a.m. to 5:30 p.m.

Our volunteers and kitchen staff serve meals buffet style. Special diets are provided as ordered.

A refrigerator is available to you in the dining room. Any food placed in the refrigerator must be labeled with your name and room number.

The dining room seating and food supplies are for patients use only. Due to our limited space, visitors are asked to wait in our lounge or the Atrium during patient meals.

Television and Telephone
Telephones are located at the patient bedside.
For television access, please call 905-821-9950 to make arrangements. This service is available weekdays from 8:30 a.m. to 5:00 p.m. Televisions are also available to all patients in the East Lounge and Dining Room.

Tours
Please come and visit us! Tours are available to you and a guest. We would love to show you around. To book a tour or for further information, please call the Clinical Practice Leader at 905-883-2414.

Discharge Guidelines
You will be assessed for home services prior to your discharge. If you require assistance after discharge, the Community Care Access Centre (CCAC) will provide services and equipment on a short-term basis for those patients who are eligible.

Assistive devices for personal needs (i.e. walkers, dressing aids) may be required on an individual basis, and should be rented/purchased prior to discharge. The therapy staff will advise you and provide information as needed.

Please note that there may be additional costs that are not covered by OHIP (ie: splints, stockings and crutches).

Discharge time is between 9:00 a.m. and 10:00 a.m.

Visiting Hours
Daytime visiting hours are flexible. Evening visiting hours end at 8:30 p.m.
