Patient and Family Information

This service is dedicated to those patients with multiple, complex care needs, whose care requirements cannot be met in one of our more specific programs. The focus in this service is on maintaining the patient’s level of activity, providing care and comfort and enhancing quality of life. A Caregiver respite service is also available (see information on Respite Care Service).

Our Philosophy of Care
Our primary goal is to improve function, enhance ability of life for al patients and their families. The focus is on care rather than a cure. Care is flexible and patient-centred. Goals are small and manageable. You and your family’s involvement are essential.

Who We Serve:

  • Patients that have multiple complex needs with no single problem being the primary focus.
  • Patients that require technology based continuing or intermittent care.
  • Patient must meet criteria for complex Continuing Care as set out by the MDS RUGS III categories.
  • Long Term Care applications/letter of agreement will be required prior to admission.

Discharge Guidelines
Our Complex Continuing Care services are a resource open to persons with complex needs rather than a final destination.

Patients are continually assessed in preparation for discharge to a more appropriate setting where their ongoing needs can be met. This depends on individual care needs.

When an application has been made to a long-term care facility the first bed to become available must be accepted in accordance with York Central Hospital’s ‘First Bed Available Policy.’

Accommodation
Both private (one bed per room) and ward (two beds per room) accommodation are available. Accommodations offered will be based on request and availability. If private accommodation has been requested, the patient will be charged the private accommodation rate from the date of admission. Otherwise the patient will be moved to a two-bed room as soon as one becomes available. Information about cost of a private accommodation is available by calling the Client Liaison at (905) 883-1212 ext. 3809.

Please note: York Central Hospital is unable to obtain information about your insurance benefits. Please ensure that you know your insurance coverage. Rehabilitation and Complex Continuing Care coverage differs from regular hospital insurance coverage. Costs incurred beyond your health care coverage will be billed to you directly.

Co-Payment Fee
Patients admitted to Complex Continuing Care pay the Chronic Co-Payment Fee in accordance with the Ministry of Health and Long Term Care. You will be required to bring in your most recent Revenue Canada Notice of Assessment.

What to Bring

  • Pajamas and housecoat. We suggest a knee length rather than floor length housecoat for ease of walking.
  • Slippers and/or flat-soled shoes.
  • Comfortable, easy dressing clothing, such as a track suit and sweat shirts
  • Radio with favourite CD's or tapes
  • Eye glasses, hearing aids, dentures
  • Any assistive devices such as your walker or cane.
  • All items except clothing need to be labeled
  • Comfort items such as pillows, blanket, pictures, etc.

Personal care products such as toothbrush, toothpaste, shampoo, Kleenex, lotions, mouthwash, and any other items you may require such as razor and sanitary napkins. For your convenience some of these items may be purchased from our Gift Shop. Please note that the unit will not supply these items.

Electrical equipment such as a razor, hair dryer, radio, etc. should be inspected by our maintenance staff. Please speak to your nurse to make these arrangements.
Please do not bring credit cards, jewelry or other valuables to the hospital. The hospital does not assume any responsibility for the loss of such items. A small amount of cash is usually sufficient for purchases from the gift shop or cafeteria.

Your Care Team
An interdisciplinary team of healthcare professionals provides the Continuing Care Program services. Your care team includes you and your family/caregiver. For more information, please refer to the Continuing Care Program brochure or hospital directory located in the patient room.

Television and Telephone
Telephones are located at the patient bedside. Patients or caregivers are required to bring their own television sets and arrange for cable hook-up with Rogers Cable. Information about cable services and the telephone number to call for hook-up arrangements is available at the Communication Centre.

Meal Times
Meals are provided in the dining room as well as the patient room, depending on the patient's condition.

York Central Hospital provides parking for our patients and visitors in a number of locations. Find out more about parking under Services and Amenities.

Visiting Hours
Daytime visiting hours are flexible. Evening visiting hours end at 8:30 p.m
