Your Guide to Recovery

Who We Are
York Central Hospital's Continuing Care Program features Complex Continuing Care, Rehabilitation and Adult Day Services. We are located in the Centre for Continuing Care and Rehabilitation (CCCR).

Continuing Care Program
Our Primary goal is to enhance the quality of life by maximizing the strengths and abilities and recognizing individual needs for all patients and their families.

It is the belief of the care team that each patient and their family should play in integral role in designing the best care options to meet the unique challenges of reintegration to the community.

Complex Continuing Care
The following services are located on level #3 of the CCCR:

Rehabilitation
The following services are located on level #1 of the CCCR:

Our focus is on care rather than a cure. Care is flexible and patient centered.
Our services are a resource open to persons with complex needs rather than a final destination.

Accommodation
Both private (1 bed per room) and ward (2 beds per room) accommodation are available. Accommodations offered will be based on request and availability.

If private accommodation has been requested, the patient will be charged the private accommodation rate from the date of admission. Otherwise the patient will be moved to a two-bed room as soon as one becomes available. Information about cost of a private accommodation is available by calling the Client Liaison at (905) 883-1212 ext. 3809.

Please note: York Central Hospital is unable to obtain information about your insurance benefits. Please ensure that you know your insurance coverage. Rehabilitation and Complex Continuing Care coverage differs from regular hospital insurance coverage. Costs incurred beyond your health care coverage will
be billed to you directly.

Your Care Team
An interdisciplinary team of healthcare professionals provides the Continuing Care Program services. Your care team includes you and your family/caregiver. Depending on the nature of your illness or injury, your personal care team may include:
Clinical Practice Leader, Registered Nurses (RNs), Registered Practical Nurses (RPNs), Patient Care Assistant, Physicians, Clinical Nurse Specialist, Geriatrician, Physiotherapist, Occupational Therapist, Speech/language Pathologist, Recreation Therapist, Social Worker, Dietitian, Diet Technician, Pharmacist, Client Liaison, and/or Wound Care Specialist.

What to Bring
Please ensure that you have the following items with you when you are admitted for care:

  • pajamas and housecoat. We suggest a knee length rather than floor length housecoat for ease of walking.
  • slippers and flat rubber soled shoes with closed heel and toes
  • stretchable socks that are easy to apply in case your foot or leg is swollen
  • comfortable, easy dressing clothing such as a track suit and sweat shirts
  • radio with favourite CD’s and tapes
  • eye glasses, hearing aids, dentures
  • any assistive devices such as your walker, cane, wheelchair or splints
  • Personal care products such as toothbrush, shampoo, tissues, razor sanitary napkins.

Please do not bring credit cards, jewelry or other valuables to the hospital. The hospital does not assume responsibility for the loss of such items. A small amount of cash is usually sufficient for purchases from the gift shop or cafeteria.
Please note that there may be additional costs not covered by OHIP i.e. splints and support stockings.

Electrical equipment such as a razor, hair dryer, radio, etc. should be inspected by our maintenance staff. Please speak to your nurse to make these arrangements.

Smoking Policy
York Central Hospital is a non-smoking facility. However, smoking is permitted outside of the hospital (9 metres from any doorway) as per Town of Richmond Hill Bylaws. Smoking is not permitted on hospital balconies. Please sign out at the Communication Centre before leaving the patient care floor.

Co-Payment Fee
Patients admitted to Complex Continuing Care pay the Chronic Co-Payment Fee in accordance with the Ministry of Health and Long Term Care. You will be required to bring in your most recent Revenue Canada Notice of Assessment. For more information please call the Client Liaison 905) 883-1212 ext. 3809.

Please note: that there are no co-payment fees for patients on level #1 Rehabilitation.

Telephones and Television
Telephones are located at the bedside in the patient rooms.

On level #1 of the CCCR (Rehabilitation Services) television rentals are available through a television provider at a daily cost. Please call (905) 821-9950 to order your television. Business hours are Monday to Friday 8:30 a.m. to 5:00 p.m.

On level #3 of the CCCR patients must bring their own television sets and arrange for cable hook up with Rogers Cable. Information and telephone numbers are available at the Communication Centre.

Visiting Hours
Daytime visiting hours are flexible. Evening visiting hours end at 8:30 p.m.

Parking
York Central Hospital provides parking for our patients and visitors in a number of locations. Find out more about parking under Services and Amenities.

Families of patients from level # 1 may purchase discounted 14 and 30 day parking tags at the parking machines.

Services Available to You During Your Stay
Café: There is a small coffee shop located on level # 1, in the Atrium (lobby) of the Centre for Continuing Care and Rehabilitation. The larger Garden Café is located in the main lobby of the Langstaff Wing.

Dental Care: This service is available at an additional cost. Please speak to your nurse.
Electrical Appliances: For the safety of our patients, the Hospital Maintenance Department must inspect all electrical appliances (hair dryers, radios, etc.) before use at the hospital.

Foot Care: This service is available at an additional cost. Speak to your nurse.

Hairdressing Salon: Operated by a professional hair stylist, the Hairdressing Salon is located off the Atrium on Level #1 in the Centre for Continuing Care and Rehabilitation. For an appointment call (905) 883-1212 ext. 3815.

Laundry: A washer and dryer is available on each level. Ask your nurse for the location. Patients and their families are responsible for personal laundry (please bring your own detergent).

Shops: The gift shop is located in the Main Lobby of the Langstaff Wing. Hospital volunteers also provide a gift cart service. The Health+ Pharmacy is also located in the Main Lobby of the Langstaff Wing.

Spiritual Care: There is a Multi Faith Meditation Room located off the Atrium on Level 1in the Centre for Continuing Care and Rehabilitation. Schedules for denominational and non-denominational services are posted on the floors and information board to the right of the elevator on Level 1, CCCR. Individual visits are also available.
