Our Commitment to Our Long Term Care Residents
The move from a house to a Long-Term Care Facility can be a difficult one – a time of major change for individuals and their families.
At York Central Hospital, we do all we can to make that transition easier. We provide exceptional care to meet each resident’s medical and personal needs.
York Central Hospital‘s Long-Term Care facility provides a warm and comfortable environment for 116 residents. Every member of our staff is committed to providing compassionate care that respects the individuality and dignity of each person in our care.
Partners in Exceptional Care
In June 2000, York Central Hospital opened the Centre for Continuing Care and Rehabilitation (CCCR). At that time, a partnership was established with Extendicare ( Canada) Inc. to provide the Long Term Care Service now located on the 4 th and 5 th floors of the CCCR.
Thanks to the professional management services provided by Extendicare, a recognized leader in the field of long-term care services for older adults, our residents benefit from state-of-the-art facilities, programs and services.
The residents of York Central Hospital’s Long Term Care Service also benefit from the close proximity to such valuable medical services such dialysis, geriatric consultation, and emergency medicine services. On-site extended medical coverage further enhances the quality of care.
Modern and Attractive Facilities
Among the newest and most beautifully appointed wings of the Hospital, the Long Term Care Service facilities are provided on the top two floors of the CCCR. This beautiful and spacious facility features:
- resident rooms with conveniently located lounges with adjoining outdoor patios that are accessible for residents and families;
- tastefully decorated rooms designed to accommodate pictures, bedspreads and small pieces of furniture from home;
- comfortable rooms with private bathrooms;
- 44 standard rooms designed for those individuals who enjoy the pleasure of one another’s company;
- delicious, nutritious meals served in brightly lit dining rooms;
- call bells in all rooms provide immediate assistance;
- Wanderguard Resident Safety System to ensure the safety of all residents: and
- Atrium Café, hairdresser and multi-faith meditation room.

Applying for Admission to Our Long Term Care Facility Anyone who is approved for Long-Term Care by the Community Care Access Centre (CCAC) (tel # 1-888-470-2222) is eligible to be a resident at York Central Hospital's Long Term Care Facility.
Co-payment and Funding Information
The Ministry of Health and Long-Term Care provides funding for long-term care facilities. The amount paid by residents for their accommodation is called a "co-payment," which varies according to the accommodation type. The Ministry of Health and Long-Term Care sets maximum accommodation co-payment rates. The following table shows the co-payment rates effective August 1, 2006. The co-payment rates change from time to time.

Government Subsidies
If your income is not sufficient to pay the accommodation co-payment for the long-stay program, there is a subsidy available to reduce the rate for standard accommodation.Residents with lower incomes or residents who have spouses or partners who are living in the community may be eligible for an additional subsidy called the "Exceptional Circumstances Reduction." For more information, you can refer to a Ministry handbook called "Accessing a Long-Term Care Facility," speak to staff at your local Community Care Access Centre or at the long-term care centre directly.
Canada Customs and Revenue Agency
Being cared for in a nursing home is considered to be a medical expense for a person with a disability and as such may qualify you for a personal tax credit. Please seek professional guidance or revue with your local office Canada Customs Revenue Agency for details. You will need to familiarize yourself with taxation form T2201, Disability Tax Credit Certificate, which can be pre-approved at anytime during the year by completing and sending to your local taxation office.
